Project Abstract

This study examined the effects of a 12-week structured resistance training program on bone mineral density (BMD) and functional fitness in older adults with metabolic syndrome (MetS). A randomized controlled trial was conducted with 120 participants aged 60–75 years diagnosed with MetS, assigned to either a supervised resistance training group (RT, n=60) or a non-exercise control group (CON, n=60). The RT program combined multi-joint exercises targeting major muscle groups, performed three times per week at moderate-to-vigorous intensity, progressing over 12 weeks to 3–4 sets of 8–12 repetitions per exercise, with loads adjusted to maintain effort near 70–85% 1-repetition maximum. Primary outcomes included lumbar spine and femoral neck BMD measured by dual-energy X-ray absorptiometry (DXA) and assessments of functional fitness using the Senior Fitness Test battery (including chair stand, arm curl, chair sit-and-reach, and timed up-and-go). Secondary outcomes encompassed body composition, blood pressure, fasting glucose, lipid profile, inflammatory markers (CRP, IL-6), and muscle strength (1RM for major lifts). Assessments were conducted at baseline, 6 weeks, and post-intervention at 12 weeks, with a 6-month follow-up for durability. Data were analyzed using repeated-measures ANOVA and mixed-effects models to account for missing data, with intention-to-treat and per-protocol analyses. The RT group demonstrated a statistically significant greater improvement in BMD at the lumbar spine and femoral neck compared with CON at 12 weeks (p<0.01), with effect sizes indicating meaningful clinical relevance. Functional fitness improved markedly in the RT group, with significant increases in chair stand and arm curl performance, improved flexibility, and faster mobility as evidenced by reduced Timed Up and Go times (p<0.01). Improvements in body composition included reduced fat mass and preserved lean mass, alongside favorable metabolic shifts such as decreased fasting glucose and improved lipid profiles (p<0.05). Inflammatory markers exhibited reductions consistent with enhanced musculoskeletal health. Subgroup analyses suggested greater BMD gains in participants with lower baseline BMD and those who achieved higher training adherence. The RT program was well tolerated, with adverse events being minor and comparable to the CON group, and adherence averaged 85%. The findings indicate that a 12-week structured resistance training protocol can meaningfully enhance bone health and functional capacity in older adults with MetS, potentially mitigating fracture risk and improving quality of life. The study supports integrating progressive resistance training into standard care for metabolic syndrome in older populations and provides a framework for scalable, community-based programs aimed at preserving skeletal integrity and functional independence. Limitations include the relatively short intervention duration for long-term BMD changes and the potential variability in DXA measurements due to positioning; future research should explore longer intervention periods, concurrent nutritional interventions (calcium and vitamin D optimization), and mechanistic investigations of muscle-bone signaling pathways in MetS.
