Project Abstract

This study evaluates a school-based physical activity (PA) intervention designed to enhance cardiorespiratory fitness (CRF) and academic performance among adolescents in rural high schools. A cluster-randomized controlled trial was conducted across 12 rural high schools over a 9-month academic year, with 6 schools assigned to the intervention and 6 to the control condition. The intervention integrated daily 20-minute moderate-to-vigorous PA bouts during homeroom and designated physical education (PE) periods, plus weekly 45-minute after-school activity clubs emphasizing aerobic games, circuit training, and walking/running programs. Implementation fidelity was monitored through weekly logs, teacher training sessions, and periodic unannounced classroom observations. Primary outcomes included CRF, measured by the 20-meter shuttle run test (beep test) and time to exhaustion on a cycle ergometer, and academic performance, assessed by mean grade point average (GPA) and standardized math and language arts test scores. Secondary outcomes encompassed body composition, self-reported physical activity levels, on-task behavior during class, attendance, and psychosocial factors such as motivation, perceived competence, and autonomous motivation for PA. Data were collected at baseline, mid-intervention (4.5 months), immediately post-intervention (9 months), and at a 6-month follow-up. Analyses employed mixed-effects regression models to account for cluster randomization and repeated measures, adjusting for baseline values, age, sex, socioeconomic status, and school-level variables. Process evaluation examined reach, dose delivered, dose received, and context to elucidate mechanisms of impact and identify barriers to implementation. Mediation analyses explored whether increases in CRF mediated improvements in cognitive function and academic performance, while moderation analyses assessed whether baseline fitness, sex, and rurality influenced intervention effectiveness. Results indicated significant improvements in CRF among intervention participants, with the intervention group showing an average increase of 2.4 stages on the shuttle run compared with 0.6 in controls (p<0.001). Time-to-exhaustion improved by 12% in the intervention group versus 1% in controls (p<0.01). Academic performance demonstrated modest but meaningful gains; the intervention group achieved higher GPA trajectories (mean difference = 0.15 grade points, p=0.04) and greater gains in standardized math and language arts scores (p<0.05) relative to controls. Secondary outcomes revealed reduced sedentary time, higher daily moderate-to-vigorous activity, and improved on-task behavior during classroom activities in the intervention group. Mediation analyses suggested that enhancements in CRF partially mediated the relationship between the PA program and academic outcomes, likely through improved executive function and mood. No adverse events related to the intervention were reported. Sustainability indicators indicated high fidelity in schools with strong administrative support and ongoing teacher professional development. These findings support the feasibility and effectiveness of integrating structured PA into rural high school schedules to bolster CRF and academic performance, with potential downstream benefits for student well-being and school climate. The study provides actionable insights for policymakers, educators, and practitioners seeking scalable, equity-focused interventions to combat rural health and educational disparities.
