Project Abstract
Degenerative joint disease (DJD) in canines imposes significant welfare burdens and economic costs, with current treatments primarily aimed at symptom management rather than disease modification. This study evaluates the efficacy, safety, and functional outcomes of mesenchymal stem cell (MSC) therapy as a disease-modifying approach for DJD in dogs. A prospective, randomized controlled trial was conducted involving 120 client-owned dogs diagnosed with radiographically confirmed DJD affecting one or more joints. Subjects were allocated to three groups (1) autologous adipose-derived MSCs administered intra-articularly, (2) allogeneic adipose-derived MSCs with immunomodulatory preconditioning, and (3) standard of care plus placebo injections. Primary outcomes included objective gait analysis, force plate metrics, range of motion, and owner-reported Canine Orthopedic Index (COI) at baseline, 6, 12, and 24 weeks post-treatment. Secondary outcomes encompassed serum and synovial fluid inflammatory markers (IL-1?, TNF-?, MMP-3), imaging-based cartilage assessment via MRI and ultrasound for a subset, and safety endpoints including adverse events, immunogenic reactions, and tumorigenicity surveillance over a 12-month period. Results demonstrated a statistically significant improvement in objective gait parameters and COI scores in both MSC-treated groups compared with placebo, with the autologous MSC group showing superior short- to mid-term outcomes at 12 weeks (p<0.05) and sustained benefits through 24 weeks (p<0.05). Allogeneic MSCs produced comparable functional gains to autologous cells, suggesting feasibility of off-the-shelf therapy with acceptable immunogenic risk, as evidenced by stable hematology and inflammatory marker profiles. Imaging assessments indicated a slower rate of cartilage degradation progression and, in a subset, preliminary evidence of neocartilage formation in MSC-treated joints. Synovial fluid analysis revealed reduced pro-inflammatory cytokines and matrix metalloproteinases post-MSC therapy, supporting a disease-modifying mechanism that includes modulation of the synovial environment and chondroprotection. Safety analysis across 12 months showed no vaccine-like adverse events, no ectopic tissue formation, and no tumorigenicity signals. The incidence of transient mild joint swelling and transient leukocytosis post-injection was low and self-limiting. Subgroup analysis suggested greater benefits in moderate (Mankin grade II) DJD compared with severe (grade III) disease, highlighting the influence of baseline joint pathology on therapeutic response. These findings support MSC therapy as a promising disease-modifying strategy for canine DJD, offering meaningful improvements in function and quality of life with an acceptable safety profile. The study provides evidence that both autologous and allogeneic MSCs can be effectively integrated into clinical practice, with consideration given to cost, availability, and immunological compatibility. Limitations include heterogeneity in DJD etiology and the relatively short follow-up for long-term disease modification; future work should optimize dosing regimens, explore combination therapies (biologic adjuncts, hyaluronic acid, and physical rehabilitation), and extend follow-up to assess long-term structural outcomes and survival.
