Project Abstract
This cross-sectional study examines the association between smartphone usage and sleep patterns, as well as behavioral health outcomes, among school-aged children aged 6–12 years in an urban (n=600) and rural (n=600) cohort, using parent-reported and child-assessed data collected over a six-month period. Sleep parameters included total sleep duration, sleep onset latency, night awakenings, and sleep efficiency gathered via actigraphy and standardized sleep diaries, complemented by the Children's Sleep Habits Questionnaire. Behavioral health was evaluated through the Strengths and Difficulties Questionnaire (SDQ) and the Pediatric Anxiety Rating Scale (PARS), along with parent and teacher reports to capture internalizing and externalizing problems, attention regulation, and mood variability. Smartphone usage was quantified in hours per day, with sub-variables for screen time after 8 p.m., social media engagement, gaming intensity, and multi-tasking behaviors, alongside parental controls and co-viewing practices. We observed a positive dose–response relationship between total daily smartphone use and delayed bedtimes, shorter total sleep duration, increased sleep onset latency, and higher night-time awakenings, with effect sizes ranging from small to moderate (? = -0.15 to -0.42 for sleep duration per extra hour of use; ORs for clinically significant sleep problems up to 1.8). Prolonged after-hours use (>1.5 hours after 8 p.m.) and high gaming intensity were most strongly associated with poorer sleep continuity and higher total SDQ scores, indicating greater behavioral problems, particularly in the hyperactivity/inattention and emotional symptoms subdomains. Sleep quality mediated a substantial portion of the relationship between evening smartphone use and behavioral problems, suggesting that disrupted sleep may be a primary mechanism linking digital exposure to mental health outcomes. Subgroup analyses indicated stronger associations in younger children (6–8 years) and in rural participants, potentially reflecting differences in parental monitoring and access to alternative activities. When controlling for physical activity and parental involvement, associations attenuated but remained significant, underscoring the independent contribution of smartphone use to sleep and behavior. The findings highlight the pervasive impact of screen-related behaviors on pediatric sleep health and behavioral functioning, with implications for pediatric guidelines, parental education, and school-based interventions aimed at promoting healthy screen habits, consistent sleep routines, and digital literacy to mitigate adverse developmental outcomes in this population. Limitations include the cross-sectional design, reliance on parent-reported measures for some variables, and potential residual confounding by unmeasured factors such as screen content type and device multitasking.
