This research project aims to investigate the impact of telehealth on chronic disease management in older adults. The aging population worldwide is facing an increasing burden of chronic diseases, leading to significant challenges in healthcare provision. Telehealth, defined as the remote delivery of healthcare services using technology, offers a potential solution to improve access to care, enhance patient outcomes, and reduce healthcare costs. Chapter One provides an introduction to the research topic, offering a background of the study, problem statement, objectives, limitations, scope, significance, structure of the research, and definitions of key terms. The growing prevalence of chronic diseases in older adults underscores the importance of exploring innovative approaches to improve disease management and overall health outcomes. Chapter Two presents a comprehensive literature review on telehealth interventions for chronic disease management in older adults. The review covers ten key areas, including the effectiveness of telehealth in managing chronic conditions, patient satisfaction, barriers to implementation, cost-effectiveness, and the role of healthcare providers in telehealth delivery. Chapter Three outlines the research methodology employed in this study, detailing the research design, data collection methods, sampling strategy, data analysis techniques, ethical considerations, and limitations.
