This research study delves into the utilization of telehealth technology in the management of chronic illnesses among elderly patients, focusing on a case study within home care nursing settings. The aim of this study is to investigate the effectiveness and feasibility of incorporating telehealth solutions to enhance the care and monitoring of elderly patients with chronic conditions, such as diabetes, hypertension, and heart disease, in home care environments. The research methodology employed a mixed-methods approach, combining quantitative analysis of patient health outcomes and qualitative feedback from healthcare providers and patients themselves. Ten home care nursing settings were selected as the study sites, each equipped with telehealth devices for remote monitoring and communication. The literature review section provides an extensive overview of existing studies on telehealth applications in chronic disease management, highlighting the benefits of remote monitoring, timely interventions, and improved patient outcomes. Various challenges and limitations in implementing telehealth in home care settings are also discussed, including technological barriers, privacy concerns, and the need for effective training and support for both patients and healthcare providers. The findings from this study reveal promising results in the use of telehealth technologies for managing chronic illnesses in elderly patients. Patients reported increased convenience, improved access to care, and greater satisfaction with the telehealth interventions.
