Telehealth, a rapidly evolving healthcare delivery model, has shown promising potential in transforming chronic disease management, particularly in elderly patient populations. This research aims to explore the impact of telehealth interventions on chronic disease management in elderly patients, focusing on the effectiveness, challenges, and implications of such technologies. The study will employ a mixed-methods approach, combining quantitative analysis of health outcomes data with qualitative insights from healthcare providers and elderly patients utilizing telehealth services. Chapter One provides an introduction to the research topic, presenting the background of the study, problem statement, objectives, limitations, scope, significance, structure, and definitions of key terms. The introduction sets the stage for understanding the importance of exploring telehealth in chronic disease management for the elderly population. Chapter Two presents an extensive literature review encompassing 10 key themes related to telehealth and chronic disease management in elderly patients. The review synthesizes existing research findings on the benefits, challenges, and best practices of telehealth interventions in this demographic group, providing a comprehensive understanding of the current state of knowledge in the field. Chapter Three outlines the research methodology, detailing the study design, sampling strategy, data collection methods, and data analysis techniques.
