This research project investigates the impact of telehealth on chronic disease management in elderly patients. With the rising prevalence of chronic diseases and the increasing aging population, there is a growing need for innovative healthcare solutions to improve patient outcomes and quality of life. Telehealth, which encompasses the use of technology to deliver healthcare services remotely, has emerged as a promising approach to address the challenges associated with managing chronic conditions in elderly patients. This study aims to explore the effectiveness of telehealth interventions in improving chronic disease management among the elderly population. The research will begin with a comprehensive literature review to examine existing studies and evidence on the use of telehealth in chronic disease management, particularly focusing on elderly patients. This review will provide insights into the current state of knowledge, gaps in research, and potential opportunities for further investigation. Subsequently, the research methodology will be outlined, detailing the approach, design, data collection methods, and analysis techniques employed in this study. The primary objective of this research is to assess the impact of telehealth interventions on key outcomes such as disease management, patient adherence, quality of life, and healthcare utilization among elderly individuals with chronic conditions. By conducting a thorough analysis of telehealth interventions and their effects on chronic disease management, this study seeks to contribute valuable insights to the growing body of literature on telehealth and geriatric care. Through a combination of quantitative and qualitative research methods, including surveys, interviews, and data analysis, this study aims to provide a comprehensive evaluation of the benefits and challenges associated with telehealth in chronic disease management for elderly patients. The findings of this research will be discussed in detail in Chapter Four, highlighting key trends, implications, and recommendations for healthcare providers, policymakers, and researchers.
