Telehealth has emerged as a promising approach to improve healthcare delivery, particularly in chronic disease management among elderly patients. This research examines the impact of telehealth interventions on chronic disease management in elderly patients from a nursing perspective. The study aims to explore the effectiveness, challenges, and opportunities associated with telehealth in enhancing care for elderly individuals with chronic conditions. A mixed-methods research design will be employed, consisting of both quantitative and qualitative data collection methods. The research will involve a systematic review of existing literature on telehealth in chronic disease management, as well as interviews and surveys with healthcare providers and elderly patients utilizing telehealth services. The findings of this study are expected to provide valuable insights into the potential benefits and limitations of telehealth in improving chronic disease management for elderly patients, thereby informing future healthcare practices and policies. By addressing the gaps in current knowledge and understanding the perspectives of both healthcare providers and patients, this research aims to contribute to the advancement of telehealth technologies and their application in caring for the elderly population with chronic diseases.
