The increasing prevalence of chronic diseases among elderly patients in nursing homes poses significant challenges to healthcare providers. Telehealth, as a technological solution, offers promising opportunities to enhance the management of chronic diseases in this vulnerable population. This research project aims to investigate the use of telehealth in chronic disease management for elderly patients in nursing homes. The study will explore the potential benefits, challenges, and outcomes associated with implementing telehealth interventions in this context. Chapter One of the dissertation provides an introduction to the research topic, including the background of the study, problem statement, objectives, limitations, scope, significance, structure of the research, and definitions of key terms. Chapter Two presents a comprehensive literature review that examines existing studies, theories, and frameworks related to telehealth in chronic disease management for elderly patients in nursing homes. The review encompasses ten critical aspects that shed light on the current state of knowledge in this field. Chapter Three outlines the research methodology employed in this study, detailing the research design, data collection methods, sampling techniques, data analysis procedures, ethical considerations, and limitations of the research methodology. This chapter comprises eight key components that provide a clear framework for conducting the research effectively. In Chapter Four, the research findings are discussed in detail, focusing on seven key aspects identified through data analysis. The discussion delves into the implications of the findings, their alignment with existing literature, and the potential impact of telehealth on chronic disease management for elderly patients in nursing homes. Finally, Chapter Five presents the conclusion and summary of the project research, highlighting the key findings, implications for practice, recommendations for future research, and the overall significance of the study. The conclusion synthesizes the research outcomes and offers insights into the potential role of telehealth in improving chronic disease management for elderly patients in nursing homes.
