Telehealth services have emerged as a promising solution to address the challenges faced in managing chronic diseases, particularly in rural communities where access to healthcare services is limited. This research project focuses on the implementation of telehealth services for chronic disease management from a nursing perspective in rural communities. The study aims to explore the impact of telehealth services on improving the quality of care and health outcomes for individuals with chronic diseases living in rural areas. The research begins with a comprehensive introduction that highlights the significance of implementing telehealth services in rural communities for chronic disease management. The background of the study provides a contextual understanding of the current healthcare landscape and the specific challenges faced by rural populations in accessing healthcare services. The problem statement identifies the gaps in healthcare delivery for chronic disease management in rural communities, emphasizing the need for innovative solutions such as telehealth services. The objectives of the study are to evaluate the effectiveness of telehealth services in improving access to care, enhancing patient outcomes, and increasing patient satisfaction in rural communities. The limitations of the study are acknowledged, including potential barriers to telehealth implementation and the need for further research to address these challenges.
