Abstract
Telehealth is increasingly being utilized in healthcare settings to improve access to care and enhance patient outcomes, particularly in chronic disease management. This systematic review aims to investigate the impact of telehealth interventions on patient outcomes in chronic disease management. The review will examine various telehealth modalities, including remote monitoring, virtual consultations, and mobile health applications, in the context of chronic conditions such as diabetes, hypertension, and heart disease. The study will include a comprehensive search of electronic databases for relevant articles published in the last decade. Eligible studies will be selected based on predetermined inclusion criteria, and data extraction will be performed using a standardized form. The extracted data will then be analyzed to identify key themes and trends in the literature regarding the impact of telehealth on patient outcomes in chronic disease management. The findings of this review will provide valuable insights into the effectiveness of telehealth interventions in improving patient outcomes in chronic disease management. By synthesizing the existing evidence, this study aims to inform healthcare professionals, policymakers, and stakeholders about the potential benefits and limitations of telehealth in the context of chronic disease management. This research is crucial in guiding the implementation of telehealth programs and interventions to optimize patient care and outcomes in the rapidly evolving healthcare landscape.
