Telehealth has emerged as a promising approach to revolutionize healthcare delivery, particularly in chronic disease management. This research aims to investigate the impact of telehealth on patient outcomes in chronic disease management. The study will explore the utilization of telehealth technologies in managing chronic diseases, such as diabetes, hypertension, and heart disease. By leveraging telehealth solutions, patients can remotely access healthcare services, receive real-time monitoring, and engage in self-management practices. The research will delve into the effectiveness of telehealth interventions in improving patient outcomes, including disease control, medication adherence, quality of life, and healthcare utilization. A comprehensive literature review will be conducted to synthesize existing evidence on the benefits and challenges of telehealth in chronic disease management. Key factors influencing the adoption and implementation of telehealth technologies will be examined, including patient preferences, provider attitudes, regulatory frameworks, and technological barriers. Methodologically, a mixed-methods approach will be employed to gather data from both quantitative and qualitative sources.
