Project Abstract

This project aims to address the significant challenge of medication non-adherence among patients with chronic diseases, which has far-reaching consequences for both individual health outcomes and the broader healthcare system. Chronic diseases, such as hypertension, diabetes, and heart disease, require ongoing management through prescription medications, yet studies have consistently shown that a substantial proportion of patients fail to take their medications as prescribed. This lack of adherence can lead to worsened health conditions, increased hospitalization rates, and higher healthcare costs. The importance of this project cannot be overstated. Chronic diseases are the leading cause of morbidity and mortality worldwide, and improving medication adherence has the potential to significantly improve patient outcomes and reduce the burden on healthcare systems. Estimates suggest that non-adherence to medication regimens accounts for up to 50% of treatment failures, 125,000 deaths, and $100 billion in avoidable healthcare costs annually in the United States alone. By developing and implementing effective strategies to enhance medication adherence, this project aims to make a meaningful impact on the lives of those living with chronic conditions. The primary objective of this project is to design and evaluate a comprehensive, multi-faceted intervention to improve medication adherence among patients with chronic diseases. The intervention will draw on the latest research and best practices in the field, incorporating strategies such as patient education, medication management tools, healthcare provider support, and targeted reminders. The project will also explore the use of emerging technologies, such as mobile health (mHealth) applications and connected devices, to enhance the delivery and effectiveness of the intervention. To achieve this objective, the project will employ a mixed-methods approach, combining quantitative and qualitative data collection and analysis. This will involve conducting in-depth interviews with patients and healthcare providers to understand the barriers and facilitators to medication adherence, as well as implementing a randomized controlled trial to measure the impact of the intervention on adherence rates, clinical outcomes, and healthcare utilization.
