Developing a Nurse-Led Chronic Disease Self-Management Program Empowering Patients, Enhancing Outcomes Chronic diseases, such as diabetes, heart disease, and respiratory conditions, have become a global health challenge, exerting a significant burden on individuals, healthcare systems, and economies. These long-term, often incurable conditions require continuous management and patient engagement to maintain optimal health and well-being. However, many individuals with chronic diseases struggle to effectively self-manage their conditions, leading to suboptimal health outcomes, increased healthcare utilization, and reduced quality of life. This project aims to address this critical issue by developing a comprehensive, nurse-led chronic disease self-management program that empowers patients to take an active role in their care. The program will leverage the expertise and patient-centric approach of nurses to provide individualized support, education, and guidance to individuals living with chronic diseases. The project will begin with a thorough needs assessment, engaging key stakeholders, including patients, healthcare providers, and community organizations, to identify the specific challenges and barriers faced by individuals with chronic diseases in the target population. This data will inform the development of a tailored self-management program that addresses the unique needs and preferences of the target audience. At the core of the program will be the nurse-led approach, which will ensure that patients receive personalized attention, evidence-based interventions, and ongoing support. Nurses, as trusted healthcare professionals with a deep understanding of chronic disease management, will work closely with patients to develop individualized care plans, set achievable goals, and provide continuous monitoring and feedback. The program will encompass a range of evidence-based components, including 1. Patient education Comprehensive, easy-to-understand education on the pathophysiology of the chronic condition, medication management, symptom recognition, and lifestyle modifications. 2.
