Project Abstract

Improving Medication Adherence Among Patients with Chronic Conditions This project aims to address the critical issue of medication non-adherence among patients with chronic conditions, which is a significant challenge in healthcare. Chronic diseases, such as hypertension, diabetes, and cardiovascular disorders, require long-term management and consistent medication intake to maintain optimal health outcomes. However, studies have shown that medication non-adherence is a widespread problem, leading to suboptimal disease control, increased healthcare costs, and a higher risk of hospitalization and mortality. The primary objective of this project is to develop and implement a comprehensive intervention strategy to enhance medication adherence among patients with chronic conditions.
