This thesis explores the impact of telehealth on chronic disease management in older adults within a long-term care facility setting. The use of telehealth technologies has gained momentum in recent years, offering innovative solutions to improve healthcare access and delivery, particularly for vulnerable populations such as older adults with chronic conditions. This research project investigates how telehealth interventions can enhance the management of chronic diseases in older adults residing in long-term care facilities, with a focus on assessing the effectiveness, challenges, and opportunities associated with telehealth implementation in this context. The study begins with a comprehensive review of the existing literature on telehealth, chronic disease management, and older adult care to establish a theoretical foundation for the research. This review identifies gaps in the current knowledge and highlights the potential benefits of incorporating telehealth technologies into the care of older adults with chronic diseases. Building upon this literature review, the research methodology section outlines the study design, data collection methods, and analysis techniques employed to investigate the impact of telehealth on chronic disease management in a long-term care facility. The empirical findings of this study are presented in the discussion section, which examines the outcomes of implementing telehealth interventions for managing chronic diseases in older adults.
