This thesis investigates the impact of telehealth on chronic disease management in elderly patients within a community setting. The study focuses on exploring how telehealth interventions can improve the quality of care and outcomes for elderly individuals living with chronic diseases. Through a case study approach, data was collected from a community healthcare facility to understand the effectiveness and challenges associated with implementing telehealth in managing chronic conditions among the elderly population.
