**Abstract
** This thesis investigates the impact of telehealth on chronic disease management in older adults. As the population ages and chronic diseases become more prevalent, finding effective ways to manage these conditions is crucial. Telehealth, the use of technology to deliver healthcare remotely, has emerged as a promising solution to improve access to care and enhance patient outcomes, particularly in the older adult population. This study aims to explore the effectiveness of telehealth interventions in managing chronic diseases among older adults and to identify the factors that influence its successful implementation. The research is guided by a mixed-methods approach, combining quantitative analysis of patient health outcomes with qualitative exploration of patient and provider perspectives on telehealth interventions. A comprehensive review of the existing literature on telehealth, chronic disease management, and older adult care forms the theoretical framework for this study. The literature review highlights the potential benefits of telehealth, such as improved patient engagement, enhanced monitoring of chronic conditions, and increased access to specialized care. The methodology section outlines the research design, data collection methods, and analysis techniques employed in this study. Data collection includes surveys, interviews, and medical record reviews to gather a diverse range of perspectives and outcomes related to telehealth interventions in chronic disease management. The research methodology aims to provide a comprehensive understanding of the impact of telehealth on older adults with chronic conditions. The findings from this study reveal the positive impact of telehealth on chronic disease management in older adults. Improved medication adherence, better symptom management, and increased patient satisfaction are among the key outcomes observed. However, challenges such as limited internet access, technological barriers, and resistance to change were also identified as barriers to successful telehealth implementation. These findings underscore the importance of addressing these challenges to fully realize the potential benefits of telehealth in chronic disease management. In conclusion, this thesis contributes valuable insights into the impact of telehealth on chronic disease management in older adults. By exploring the effectiveness of telehealth interventions and identifying key factors that influence their success, this study provides actionable recommendations for healthcare providers, policymakers, and researchers to enhance the delivery of care for older adults with chronic conditions. The findings from this research have the potential to inform future telehealth initiatives and improve health outcomes for older adults worldwide.
