The utilization of telehealth in healthcare has revolutionized the delivery of services, particularly in managing chronic diseases among elderly patients. This study explores the impact of telehealth on chronic disease management in the elderly population, focusing on its effectiveness, challenges, and potential benefits. Through an in-depth literature review, key themes and findings related to telehealth in chronic disease management are synthesized and analyzed. The research methodology involves a mixed-methods approach, including surveys, interviews, and case studies with healthcare providers and elderly patients utilizing telehealth services. Chapter One provides an introduction to the study, presenting the background, problem statement, objectives, limitations, scope, significance, structure of the thesis, and definition of key terms. Chapter Two offers a comprehensive literature review encompassing ten key areas related to telehealth in chronic disease management among elderly patients. Chapter Three outlines the research methodology, including the research design, data collection methods, sampling techniques, data analysis procedures, ethical considerations, and limitations. Chapter Four presents a detailed discussion of the findings, highlighting the impact of telehealth on chronic disease management in elderly patients, barriers to implementation, patient satisfaction, healthcare provider perspectives, and best practices. The chapter also explores the role of technology, patient education, and remote monitoring in enhancing healthcare outcomes for elderly individuals with chronic conditions. In Chapter Five, the conclusion and summary of the project thesis are provided, offering insights into the implications of telehealth for chronic disease management in the elderly population.
